400 Fewer Falls Initiative

The 400 Fewer Falls initiative is a free program focused on reducing the fall risk of 400 seniors on Long Island within the next 12 months. We provide a free in-home balance assessment and fall risk analysis to address the internal obstacles and environmental factors that increase falls in the home.
Daniel Davids PT DPT CSCS is the owner of The Traveling Physical Therapist, a Medicare-covered in-home PT service covering Long Island and most of Queens.

Andrea Casey has been a Proud Rotarian since 1999 with the Sayville Rotary Club. I am a Past President of the Sayville Club , served as Co Hospitality Chair for Past District Govenor Shawn Weis at his district conference this past May. My professional background is as Maternal Child Health Specialist/ Registered Nurse and Perinatal Educator since 1987, and a Licensed Realtor since 1998. When I retired from nursing 6 years ago I decided to return to my true passion, Horses. I began training once again with horses 6 years ago and eventually sought out Pal-O-Mine Equestrian as a Volunteer. I am proud to say that it has been a great ride and a few years ago I became Pal-O-Mine’s Volunteer Coordinator. I am very excited to share a bit of Pal-O-Mine Equestrian and all that we do. I look forward to be the speaker at Commack, Kings Park Rotary.

The Motivation Method: Strategies for Unstoppable Motivation Before she was a motivational maven, Corrie LoGiudice experienced this often. In a span of 4 years, she experienced miscarriage, abuse, divorce and suicide loss. What she’s learned through her own experience is growth stagnates when you don’t change your strategy to accommodate your current reality. She eventually figured out that there are 5 areas that if out of alignment will slow your motivation. When she strategized how to address the imbalances, both hers and her client's motivation becomes unstoppable. She calls it the Motivation Method, and in this talk she summaries how it works and how you can start using it today. This keynote is humorous, honest, and motivational. The audience will not only be captivated but will leave the event feeling more confident in their own abilities and prepared to go get their goals. Corrie LoGiudice is a motivational strategist & speaker, host, influencer and motivational maven. She’s a former SVP, single mom, and survivor of miscarriage, divorce, abuse & suicide loss. She utilized her trauma to fuel her motivation instead of set her back, and now teaches others to do the same.

Camp Good Mourning

Camp Good Mourning! is a Huntington-based 501c3 nonprofit organization that provides FREE overnight bereavement camp programs for Long Island children who have experienced the death of a parent and/or sibling.
During the presentation, I will share specific information about:
• Childhood grief
• What makes Camp Good Mourning! unique
• How grieving children and their families benefit from Camp Good Mourning!
• Why Camp Good Mourning! is important to the Long Island community
• The benefits of supporting grieving Long Island children and Camp Good Mourning!
Paul is the Executive Director of Camp Good Mourning!, a Huntington-based 501c3 nonprofit that provides FREE bereavement camp program experiences for Long Island children coping with the death of a parent and/or sibling. Paul has 17 years of bereavement camp experience -- providing services on both east and west coasts -- with children, adolescents, teens, and their families. He also serves as a Support Group Leader for bereaved widows and widowers at the SYJCC and as a Pro Bono Support Group Leader for Life after Loss, a Nassau County nonprofit organization for families who have lost loved ones to gun violence. Paul has been recognized by the Nassau County Executive for his work as a Hurricane Sandy Crisis Counselor and honored by Suffolk County officials for his work with Long Island students in the area of Bully and Cyber Bully Prevention and Intervention. He earned his Master’s Degree in School Counseling and an Advanced Certificate in Mental Health Counseling from LIU Post. He is a provisionally certified School Counselor in NY, a certified School Counselor in NJ, and a Clinical Mental Health Counselor-LP specializing in grief and loss.

Camp Pa Qua Tuck

Bridget Reilly Costello joined Camp Pa Qua Tuck in 2009 as the Summer Director. In 2010, she became the Executive Director. In April, 2019 Bridget retired from her position at the Camp but was delighted that the Board of Directors embraced her idea of P/T work promoting the camp as she eased her way into retirement. Bridget was raised with a Special Needs brother and is his Guardian. This had more to do with her decision to pursue advocacy for individuals with Special Needs than anything else.
Proud mother of 3, wife to Michael, a Veteran and a Purple Heart recipient, Bridget volunteers at the V.A. Hospital in Northport. Additionally, one of her children was born with a non-functioning liver and Bridget was able to donate a portion of her liver to save her baby 20 years ago. As a result, she has been a member of the Alpha 1 Foundation for 20 years and has traveled all over to speak to others facing this life-changing situation.
Bridget spent 30 years in the non-profit world and has observed the best practices in several large non- profits. Her goal has been to bring those best practices to Camp Pa Qua tuck. Bridget is also a member of AFP, the Association of Fundraising Professionals. She has great regard for her predecessors at camp and hopes that she can carry on the legacy of continuous improvement that they have initiated.

There will be a panel discussion to answer your questions about starting a business followed by a networking event to connect with other local businesses. Meet the professional and community leaders in Rotary, as well as many other professionals from all over Long Island. It is $5 in advance or $10 at the door. Come out, meet new friends and colleagues and enjoy some light refreshments on us. Coffee and snacks will be served and a directory of all the attendees (who RSVP'd at least 5 days in advance) will be provided.There will also be a door prize. There is an opportunity to advertise in the attendee directory, and business card sized ads (which will be printed in color) are available for sale at $20 each. Limit of 2 adjacent ads per attendee/company and a total of only 10 will be sold. Ads will only be accepted until 7 days prior to the event.

LI Against Domestic Violence

We are proud be serving Long Island for over 40 years. We also take pride in doing things a bit differently than others including providing comprehensive and supportive services including hotline, counseling, vocational training, emergency shelter, prevention and educational resources and legal advocacy. I’ve attached a digital version of our services brochure for some quick reference as well. All of our services are also available in to the Spanish speaking population. Our formal mission statement: LIADV is committed to the empowerment of victims and survivors of domestic violence through supportive services including hotline, counseling, vocational training, emergency shelter, prevention and education resources and legal advocacy. In addition to our foundational services we have many programs that set us apart, in which we are the only providers. For example, we are the only agency to have a direct and formal relationship with Suffolk County Police, District Attorney’s office and probation. This allows for a safety net immediately following an incident which has proven to reduce additional violent incidents in Suffolk County each year for the last five years.

As an Independent representative, Gwen has worked in the insurance industry for over 25 years and brings a wealth of knowledge to Medicare Health Insurance. She is familiar with local insurance options, as well as assistance and savings programs. Gwen helps seniors navigate Medicare, find plans to meet their needs and budget, provides updated information on Medicare and education on local and national Medicare entitlements. She also hosts seminars and adult education classes on the topic of Medicare to help seniors understand what their options are. No question is too small! Gwen understands that everyone has different needs and different financial obligations that can influence their plan choices. She works with seniors and presents options when exploring Medicare Health and Prescription Drug Plans, and well as Medicare/Medicaid plans. Gwen understands that the Medicare Insurance Marketplace can be overwhelming. As a licensed representative, it is her responsibility to stay current on the changes that are happening with Medicare.

Camp Kesem

The speaker is the active Co-Director of Camp Kesem at Stony Brook University. He is a senior who is receiving his BA in Political Science and aspires to go to medical school. He also works in a psychology lab to better understand kids with autism. In his free time he volunteers at the Stony Brook Hospital Child Life Department. Camp Kesem is an active community throughout the United States, driven by college students, supporting children through and beyond their parent's cancer with free year-round services. Camp Kesem's main program is a week-long sleepaway summer camp for kids whose parents have or have had cancer.

Quest Nutrition Network

John Jabari Michel is a nutritionist and health & wellness professional with over 17 years of experience, education and expertise. Jabari formed Quest Nutrition Network in 2012 with goals of assisting clients in achieving optimum health through nutrition, exercise, meditation and mental health. Jabari spreads the message of Optimum Health through his speaking engagements, social media platforms such as Facebook, Instagram and You tube, as well as one on one coaching consultations. Jabari began his journey into nutrition as a result of growing up in a family with health challenges such as overweight/obesity, diabetes, cancer and heart disease. Having witnessed several family members and close friends succumb to untimely deaths due to poor health and nutrition habits, Jabari directed his time and energy towards educating himself on nutrition and health. At age 19 he removed Pork, Beef and Chicken from his diet and instead of cow’s milk, he opted with almond milk. Upon completion of his bachelor’s degree in Nutrition in 2006, Jabari later re-enrolled into CUNY Queens College to complete his master’s degree in Nutrition and Exercise Science in 2016.

Rob Raylman has held the position of Chief Executive Officer of Gift of Life International since February 2008. He has been a member of the Manhasset Rotary since March 2008. Before Rob came to Gift of Life International he was a Vice President for Browning-Ferris Industries (BFI), at one time the second largest solid waste management company in the world. He left BFI in 1999 and started his own consulting business in upstate New York, where he represented individuals and corporations that did business in New York State. He specialized in legislative and regulatory affairs. Rob is a life-long resident of New York. He graduated from Hobart College in 1984 with a BS in Political Science. Directly out of college Rob went to work for United States Senator Alphonse D’Amato (R-New York). He worked from 1984 to 1986 for the Senator. At which time he began work for BFI in January of 1987 until 1999. Since Rob began as Chief Executive Officer of Gift of Life International he has been focusing on the development of sustainable pediatric cardiac surgery programs in emerging countries. Presently he is working to develop programs in the Jamaica, The Philippines, El Salvador, Uganda, and Bolivia. Each of these programs is in various stages of development. Rob lives in Germantown, New York.

Maureen joined Birthday Wishes in 2013 after a career in sales. She has always wanted to make a difference, and Birthday Wishes grants her that opportunity. She gets tremendous pleasure in fulfilling the wishes of birthday children. She is married to Frank and they have two sons, Joseph and Robert. In her free time, Maureen enjoys crafting and finding antiques at flea markets. The mission of Birthday Wishes is to improve and empower the lives of homeless children and their families by providing joy, play, and hope through the magic of a birthday party. Our goal is to ensure that children experiencing the trauma of homelessness have the opportunity to celebrate their birthday joyfully, surrounded by family, friends and others who care.

Suffolk County Council Boy Scouts of America

The Boy Scouts of America (the BSA) is one of the largest Scouting organizations in the United States of America and one of the largest youth organizations in the United States, with more than 2.4 million youth participants and nearly one million adult volunteers. The BSA was founded in 1910, and since then, more than 110 million Americans have been participants in BSA programs at some time. The BSA is part of the international Scout Movement and became a founding member organization of the World Organization of the Scout Movement in 1922.
